Our experienced care team is dedicated to providing tailored support, guidance and encouragement as needed, helping our residents to reach their full potential and make a positive impact in their communities.
Supporting Independence
We aim to provide a home-like environment where people with special needs can live independently, but also be supported in being active and engaged as possible while ensuring their safety, health, and emotional well-being. One of our key objectives is to facilitate a smooth transition from family or previous care settings, keeping them in touch with loved ones and maintaining ongoing relationships. We encourage residents to pursue their individual interests and participate in the local community.
Through this support, we aim to promote social benefits and enjoyment, allowing individuals to reap the rewards of house-sharing with others who share similar experiences and passions. Each resident is assigned a key worker who oversees their daily routines and nurtures personal development, creating a tailored approach that addresses their unique needs and goals.
Individual Assessments
When selecting the best living environment for your loved one, it's essential to consider their unique needs and circumstances thoroughly. Our comprehensive assessment process takes into account every aspect of each resident's profile, from their functional abilities to their social support networks. This involves an initial in-person meeting to build trust and establish a relationship with them, which is then reviewed periodically to ensure the assessment remains relevant and tailored to their changing needs.
We take great care to conduct our assessments in person, whenever possible in the resident's current living environment, where they feel comfortable, secure, and able to make informed decisions. By doing so, we gain valuable insights into their daily functioning at home, including areas of strength and improvement, which informs our ongoing support and guidance as they navigate this new chapter.
Once a resident has been accepted into care, we collaborate with other medical professionals, therapists, dieticians, peers, and palliative care specialists to ensure seamless coordination of services. This collaborative approach allows us to provide comprehensive care that addresses the diverse needs of each individual, while also respecting their autonomy and preferences.
